Key Responsibilities\n

• Conduct detailed psychosocial assessments to evaluate the behavioral health needs of patients, facilitating the identification of diagnoses and the formulation of comprehensive care plans.

• Develop, implement, and monitor individualized treatment plans in collaboration with the medical team, ensuring responsiveness to the evolving needs of each patient.

• Utilize your knowledge of cultural competency to provide appropriate interventions tailored to the specific needs of patients across various life stages, including children through to the elderly.

• Engage in effective communication with physicians and other healthcare professionals about patients' behavioral health challenges and coordinate referrals to community resources when necessary.

• Uphold a patient-centered environment, ensuring that all interactions prioritize safety and adhere to best practices, reducing the likelihood of healthcare errors.

Patient Population / Client Focus\n

Your primary focus will be on patients in hospice care, many of whom are facing significant emotional and psychological challenges related to terminal illness. You will be instrumental in supporting these patients and their families through grief, loss, and the complex emotions that arise at the end of life. This role requires sensitivity, compassion, and a deep understanding of the unique needs of this vulnerable population.

Qualifications\n

• Master’s degree in Social Work (LICSW) or a related field from an accredited institution.

• Current and valid licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington State.

• A minimum of two years of professional social work experience, specifically in psychiatric settings or hospice care.

• Current certification or training in the safe management of disruptive or assaultive behavior.

Compensation and Benefits\n
