Key Responsibilities\n

As an LCSW reporting to the Social Work Manager, you will be responsible for:

• Evaluating and diagnosing a variety of psychosocial issues impacting patients and their families, employing individual clinical social work skills to enhance their psychosocial functioning.

• Conducting comprehensive bio-psychosocial evaluations to identify treatment obstacles and optimize patients' health statuses.

• Collaborating with multidisciplinary team members to develop and execute individualized treatment plans, utilizing evidence-based treatment modalities that best suit each patient’s needs.

• Documenting patient care activities in compliance with departmental standards while adhering to guidelines set forth by the National Association of Social Workers (NASW) Code of Ethics.
