Key Responsibilities\n

• Deliver personalized counseling sessions addressing the unique mental health needs of clients, employing clinically approved methodologies and therapeutic interventions.

• Facilitate transportation and assist clients with navigating public transportation options, ensuring they maintain access to critical health appointments.

• Proactively monitor clients’ supply needs, ensuring they have access to essential household items required for their well-being.

• Commit to providing the highest quality of care, tracking client progress against established goals while ensuring their satisfaction and mental health needs are met.

• Maintain compliance with all legal, regulatory, and contractual requirements, ensuring adherence to program certifications and licensing mandates.

• Work collaboratively within your team by sharing information and resources, contributing to a cohesive and effective service delivery model.

• Document all client interactions and maintain organized records, including notes and reports on client progress.
\n
• Guide clients through daily living activities, offering support and teaching skills necessary for independent living.

• Monitor client mood and behavior, reporting pertinent observations to ensure a safe and supportive environment.

• Assist clients in meal planning and preparation, fostering life skills that promote healthy and independent lifestyles.

• Provide medication reminders and assist clients in managing their medications, ensuring adherence to prescribed treatment regimens.

Qualifications\n

• A Master’s degree in counseling, social work, clinical psychology, marriage and family therapy, or a related field is required.

• Active licensure in the state is a must—LCSW, LMFT, or LPCC. Note that associates’ licenses are not eligible, as they cannot be billed through Medicare.

• Exceptional proficiency in computer skills, particularly with Microsoft Office Suite, to facilitate effective documentation and client management.

• A capacity to thrive in a fast-paced environment, handling multiple cases with professionalism and efficiency.

• Strong self-management skills, with an ability to work autonomously without direct supervision.

Preferred Experience\n
