Key Responsibilities\n

• Proactively engage with clients to develop therapeutic strategies that minimize the need for crisis services.

• Collaborate with county crisis services to create and maintain crisis alerts that ensure adequate support for participants during emergent situations.
\n
• Document and coordinate seamless transitions for clients to hospital or inpatient services, including liaising with medical staff to facilitate effective discharges.
\n
• Monitor and evaluate patient progress, adjusting treatment plans as necessary to align with their evolving needs.
\n
• Conduct thorough assessments, develop personalized care plans, and implement evidence-based interventions to address mental health issues.

Patient Population / Client Focus\n

As a Telehealth Professional Counselor, you will work with a diverse range of clients, each presenting unique psychiatric, emotional, and social challenges. The population you serve will include individuals experiencing anxiety, depression, trauma, and other mental health disorders, requiring cultural sensitivity and a tailored approach to communication and interaction with clients and their families.

Qualifications\n

• A Master’s degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling.

• An active state license as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Counselor (LPC).

• Strong foundational knowledge in various therapeutic modalities and mental health treatment strategies.
