Key Responsibilities\n

• Conduct assessments during initial client sessions to understand individual goals, preferences, and available choices, leading to the development of personalized rehabilitation plans.

• Draft tailored treatment strategies focusing on clients' educational background, skills, abilities, and career aspirations, while adjusting these plans throughout the rehabilitation process as necessary.

• Collaborate continuously with a multidisciplinary team consisting of physicians, occupational therapists, psychologists, and other professionals to design and implement customized rehabilitation programs for each client.

• Maintain thorough and accurate client records, documenting personal information, eligibility data, insurance details, services rendered, and ongoing communication.

• Regularly monitor client progress towards their goal achievements, documenting outcomes and making timely interventions as required for success.

• Ensure consistent interaction with clients participating in job training and related programs, aiding them in resolving any challenges they encounter.

• Engage with clients' family members to explore and implement effective strategies for their support.
\n
• Perform needs assessments promptly and create documentation to reflect findings accurately.

• Provide resources and referrals as needed to support clients in achieving their therapeutic and life goals.

Qualifications\n

• A Master’s degree in Counseling, Psychology, Social Work, or a related field.
