Key Responsibilities\n

• Provide intensive case management services tailored to a focused caseload of children and families, ensuring personalized support for each individual.

• Assess and respond to urgent care needs, triaging effectively to prioritize interventions.

• Support families in navigating paperwork by assisting with the completion of necessary forms and applications related to mental health services.

• Identify and address social determinants of health (SDOH) that may impact your clients' mental health and overall well-being.

• Facilitate connections for families to essential community resources and support services.

• Track referral processes and provide follow-up to guarantee that service delivery is effective and timely.

• Assist patients in scheduling and attending medical, behavioral health, and specialist appointments to ensure continuity of care.

• Provide essential administrative support, including the retrieval and review of records, eligibility screening, and monitoring of client goals.

• Coordinate comprehensive care for each patient, ensuring that all aspects of their health needs are addressed.

• Conduct home and community visits as necessary to better understand and meet family needs.
\n
• Collaborate with interdisciplinary teams, including behavioral health providers and external partners, to ensure coordinated care.

• Document all patient interactions and services in compliance with organizational protocols using electronic health records.

• Stay actively engaged in meetings and training sessions, contributing to the continuous professional development of your knowledge and skills.

Patient Population / Client Focus\n

Your primary focus will be children and their families facing mental health challenges within the school environment. Qualifications\n

• Active LCSW license in Pennsylvania.

• Bachelor's degree in Social Work (BSW) is required; a Master’s degree in Social Work (MSW) from an accredited program is preferred.

• A minimum of 2 years of relevant experience in clinical settings, particularly within mental health or social services.

• Proficiency in evidence-based therapeutic interventions such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), or Eye Movement Desensitization and Reprocessing (EMDR).

• Strong skills in case management and crisis intervention, with the ability to assess urgent situations effectively.

• Excellent communication and interpersonal skills to build rapport with clients, families, and colleagues.

Compensation and Benefits\n
