Key Responsibilities\n

• Conduct thorough and ongoing assessments to identify and address the specific needs of patients, tailoring treatment plans accordingly.

• Serve as a critical communication link among supervisors, direct care staff, families, and ancillary support personnel to ensure holistic care and monitor client progress.

• Develop and share tentative schedules with supervisors for intake sessions, home visits, and comprehensive assessments to facilitate seamless service delivery.

• Coordinate and perform home visits, chart reviews, and data analysis that inform ongoing treatment and foster a community-based approach to care.

• Implement and monitor individualized treatment plans, ensuring they are executed effectively and adjusted as necessary based on client feedback.

• Participate actively in interdisciplinary team meetings, contributing insights and strategies that align with patient care goals.

• Document progress through detailed monthly and quarterly reports, effectively communicating outcomes to relevant parties.

• Organize training sessions for staff to enhance their competency in implementing treatment plans and maintaining documentation standards.

• Offer follow-up support through interviews and subsequent home visits to ensure clients receive ongoing assistance based on their evolving needs.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required to ensure a strong foundation in mental health practices.
