Key Responsibilities

• Conduct comprehensive evaluations of patients by reviewing medical histories, performing assessments, and documenting findings to formulate effective treatment plans.
• Communicate evaluation results and proposed treatment strategies clearly to patients and their families, ensuring their involvement in goal-setting discussions.
• Implement individualized treatment protocols based on patient evaluations, current presentations, and established best practices in physical therapy.
• Collaborate with multidisciplinary healthcare teams as needed, documenting any changes in patients' conditions and adjusting care plans accordingly to enhance recovery outcomes.
• Manage discharge planning effectively, including follow-up care consultations, while ensuring accurate documentation of all patient interactions, progress notes, and treatment activities in compliance with regulatory standards.
• Maintain a productive schedule while utilizing efficient time management techniques; contribute to departmental goals and participate in marketing initiatives to promote the clinic's services.
• Engage in continuous learning and professional development opportunities and serve as a mentor for students in clinical placements.
• Actively support initiatives aimed at improving access to care and enhancing patient experiences through adaptability and responsiveness to patient needs.

Patient Population / Client Focus

You will work primarily with a diverse range of patients requiring home-based therapy, including the elderly, stroke survivors, individuals recovering from surgery, and those managing chronic pain conditions. This role allows you to observe firsthand how therapeutic interventions can significantly improve patients' quality of life, regain mobility, and enhance their overall well-being.

Qualifications

• A Bachelor's Degree in Physical Therapy (Doctorate preferred) with appropriate licensure in the state of Virginia.
• A valid license to practice as a Physical Therapist in Virginia.
• Current American Heart Association's BLS for Healthcare Providers CPR certification is required.
• Strong interpersonal and communication skills to foster effective relationships with patients and team members.

Preferred Experience

Compensation and Benefits
