Key Responsibilities\n

In the role of Licensed Clinical Social Worker, you will:

• Conduct thorough psychosocial assessments to identify and address the emotional, social, and economic needs of hospice patients.

• Provide both individual and group counseling sessions tailored to enhance coping skills, address grief, and facilitate adjustment to end-of-life issues.

• Support families by offering emotional education regarding treatment choices, resources available, and what to expect moving forward.

• Collaborate with the interdisciplinary team to devise comprehensive care plans, ensuring an integrated approach to patient care.

• Facilitate referrals to community resources and post-discharge services that promote seamless transitions and ongoing support.

• Participate in organizational committees where your expertise on social work matters contributes to enhancing service delivery and patient care standards.

• Maintain meticulous and timely documentation compliant with legal and hospital policies, ensuring all records are up-to-date.

Patient Population / Client Focus\n

Your primary focus will be on patients in hospice care who are experiencing life-limiting illnesses. This vulnerable population requires empathy and understanding, along with skilled intervention to navigate the complexities of emotional distress associated with end-of-life scenarios. You will also work closely with families, helping them cope with their loved ones' illnesses, changes in family dynamics, and preparations for eventual loss.

Qualifications\n

To be considered for this role, candidates should possess the following qualifications:

• Master’s degree in Social Work from a program accredited by the Council on Social Work Education (CSWE).

• Active licensure as a Licensed Clinical Social Worker (LCSW) within the state of Oregon.

• Strong understanding of mental health disorders, family systems, and crisis intervention.
