Key Responsibilities\n

• Conduct comprehensive assessments and develop individualized treatment plans that address the specific needs of each client.

• Maintain ongoing communication with supervisors, support staff, and family members to monitor client progress and adjust treatment strategies accordingly.

• Organize and oversee home visits as well as therapy sessions to ensure alignment with treatment goals and compliance with program plans.
\n
• Document progress notes and other relevant data monthly and quarterly, ensuring transparency and accessibility of information for future reference.

• Collaborate within an interdisciplinary team to provide holistic care and participate in team meetings to discuss patient cases, treatment updates, and strategies for improvement.

• Act as a liaison between clients and the broader mental health community, facilitating access to additional services and resources.
\n
• Train and support fellow counselors and staff in best practices, program implementations, and documentation standards.
\n
• Provide follow-up care through interviews and observations, ensuring that clients receive continuous support in their mental health journey.

• Take an active role in community outreach to promote mental wellness and educate the public on available resources.

Patient Population / Client Focus\n

Your practice will involve working with a diverse range of clients, including adults and youth facing various mental health challenges such as anxiety, depression, trauma, and relationship issues. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Current, unrestricted licensure as a Professional Counselor (LPC) in Colorado.

• A minimum of two years of relevant counseling experience is essential, preferably in a clinical setting.

• Strong foundational knowledge of legal and ethical standards in mental health practice.

• Excellent communication and interpersonal skills, with a proven ability to build rapport across diverse client populations.

• Previous experience collaborating effectively within healthcare teams.

• Familiarity and proficiency with Electronic Medical Records (EMR) systems and related software.

Compensation and Benefits\n
