Key Responsibilities\n

• Conduct thorough assessments and evaluations of patients to establish and tailor individualized treatment plans that incorporate culturally diverse considerations and address special needs.

• Utilize the Diagnostic and Statistical Manual of Mental Disorders (DSM) and other clinically endorsed assessments to effectively diagnose mental health conditions when necessary.

• Monitor patient progress, implementing evidence-based interventions and modification strategies to enhance treatment outcomes continually.

• Document patient interactions meticulously, ensuring treatment records and clinical information are updated accurately and on time, aligning with regulatory standards and best practices.

• Collaborate proactively with patients and their families to co-create comprehensive treatment plans that prioritize patient goals and desired outcomes, enhancing the overall care experience.

• Link patients to necessary community resources and services, while building and maintaining strong relationships with other healthcare providers for coordinated care.

• Facilitate education and mentorship for emerging professionals, including students and interns, while participating in ongoing professional development opportunities.

• Engage in clinical team meetings and offer peer review, chart auditing, and collaborative support as directed by clinical supervisors to propel team success and cohesion.
