Key Responsibilities\n

• Conduct thorough assessments of counseling needs for clients, focusing on their overall mental health and substance use.
\n
• Utilize the DSM (Diagnostic and Statistical Manual) criteria to assess clients for substance abuse diagnoses, determining appropriate levels of care using ASAM (American Society of Addiction Medicine) standards.
\n
• Prepare and implement substance abuse counseling components within individualized client treatment plans.
\n
• Develop and maintain discharge and continuum of care plans to facilitate smooth transitions to appropriate care levels.
\n
• Document all counseling, educational services, assessments, and follow-ups within client clinical records accurately and consistently.
\n
• Manage a caseload that includes comprehensive case management, treatment planning, and clinical documentation as needed.
\n
• Provide coordination of support services for clients and their families, ensuring comprehensive records of these services in clinical files.
\n
• Maintain contact and communicate effectively with referral sources, such as legal establishments, educational institutions, and employers to support client needs.
\n
• Facilitate crisis intervention as needed, employing a multidisciplinary team approach to address immediate concerns for assigned clients.

Patient Population / Client Focus\n

In this role, you will work with a diverse array of clients primarily facing challenges related to substance use disorders and co-occurring mental health conditions. Your expertise will directly contribute to establishing therapeutic relationships that foster healing and long-term recovery. You will work closely with individuals and families, facilitating group and individual therapy sessions aimed at building resilience and recovery foundations.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Current LCSW licensure in the state of Texas.

• Strong skills in building therapeutic relationships, demonstrating professionalism, and maintaining ethical boundaries.

• Cultural competency to work effectively with diverse populations.

Preferred Experience\n

• Experience in co-occurring counseling and addiction recovery settings.

• Experience in utilizing electronic health record (EHR) systems for documentation and reporting.
