Work Environment\n

This role allows for a fully remote work setting, offering flexibility to manage your schedule while focusing on providing support to clients. The standard work hours are from 9 AM to 6 PM, Monday through Friday, with no evenings or weekends required. Key Responsibilities\n

• Conduct thorough assessments of mental health needs, develop personalized treatment plans, and coordinate and implement behavioral health services tailored to each client’s requirements.

• Maintain a deep understanding of the current trends and regulations within local, state, and federal behavioral healthcare programs to effectively guide clients.
\n
• Collaborate as a member of a multidisciplinary primary care team, engaging with all levels of staff and primary care providers to ensure comprehensive patient care.
\n
• Adhere to and maintain mental health policies and procedures while actively participating in the ongoing development of our behavioral health program.
\n
• Provide timely intakes and assessments, ensuring that each patient feels welcomed and understood from their first interaction.
\n
• Offer crisis intervention and counseling services to clients experiencing immediate mental health issues, utilizing mature judgment and evidence-based practices.
\n
• Refer patients with severe mental health conditions requiring inpatient or specialized care to the appropriate institutions or agencies.
\n
• Implement screening for prescription and illicit drug abuse, as well as alcohol misuse, and provide counseling or refer clients to relevant local resources.

Patient Population / Client Focus\n

The ideal candidate will support a diverse clientele, many of whom are individuals dealing with chronic illnesses or complex psychosocial needs within the hospice setting. You will play a critical role in advocating for these patients, helping them to find the right resources and support systems as they navigate their healthcare journeys.

Qualifications\n

• A Master’s degree in Social Work or a closely related field from an accredited institution.

• Current LCSW licensure allowing practice in Connecticut.

• Demonstrated clinical excellence with a proven track record of effective therapy delivery.

• Ability to maintain mature judgment in high-pressure situations, particularly during crisis moments.

• Strong communication skills, including aptitude for articulating information clearly in both oral and written formats.

• Proficiency with electronic health record systems and the capability to manage patient documentation efficiently.
