Key Responsibilities\n

As a Licensed Professional Counselor at Thrive Hard, you will:

• Collaborate with primary care providers to identify and manage mental health concerns, becoming an integral member of the patient care team.

• Conduct comprehensive patient intakes, performing thorough diagnostic and functional evaluations to inform treatment goals.
\n
• Deliver brief, focused interventions for clients requiring mental health services, utilizing evidence-based practices tailored to individual needs.
\n
• Advise primary care providers on whether patients should remain in the primary care setting or be referred to specialty mental health facilities.
\n
• Ensure effective patient follow-up to monitor progress and adapt treatment plans as necessary.
\n
• Develop relapse prevention strategies to help patients maintain stability and encourage better lifestyle choices.
\n
• Assist in identifying high-risk patients, working collaboratively to create pro-active plans addressing their mental health needs.
\n
• Coordinate behavioral health service delivery, linking patients with other treatment providers both within and outside the primary care network.

• Maintain up-to-date and accurate documentation of patient progress, treatment plans, and diagnoses in accordance with agency policies.
\n
• Engage in program evaluation discussions with management, providing insights on behavioral services and suggested improvements.

• Guide patients through medical treatments initiated by primary care providers, equipping them with coping strategies for potential side effects.

• Uphold the Code of Ethical Conduct for Clinical Professionals in all interactions.

• Perform additional duties as needed to support clinic operations and patient care.

Qualifications\n

• A Master’s Degree in Clinical Counseling or a related field is required.

• A current LPC (Licensed Professional Counselor) license to practice in Pennsylvania is essential.

• Familiarity with developmental psychopathology and psychodiagnostics is strongly preferred.

Preferred Experience\n
