Key Responsibilities\n

• Conduct thorough mental health assessments, including psychosocial evaluations and risk assessments, to develop a clear understanding of each patient’s challenges.

• Collaborate with patients and their families to formulate personalized treatment plans that incorporate their specific goals, values, and cultural backgrounds.

• Deliver a range of therapeutic interventions, such as Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), and Solution-Focused Brief Therapy (SFBT), across various formats: individual, couple, family, and group therapy sessions.

• Provide crisis intervention services, ensuring patients receive timely support, and work closely with other healthcare providers to maintain continuity of care during times of acute distress.

• Maintain comprehensive clinical documentation, including progress notes, treatment plans, and discharge summaries, while adhering to regulatory guidelines and clinic policies.

• Educate patients and their families through psychoeducation, equipping them with vital tools to manage their mental health effectively and fostering greater self-sufficiency.

• Engage in regular meetings with a multidisciplinary team encompassing psychiatrists, nursing staff, and case managers to ensure coordinated care for all patients.

Patient Population / Client Focus\n

The LCSW will work with a diverse patient population at a Hospice Center in Escondido, California. Your expertise will be crucial in addressing the unique mental health challenges faced by individuals in palliative care settings, including grief counseling, coping with chronic illness, and supporting families through end-of-life transitions. By applying culturally-sensitive approaches, you will ensure that care is respectful and responsive to the varied backgrounds of patients and their loved ones.

Qualifications\n

• A Master’s degree in Social Work is required.

Compensation and Benefits\n
