Key Responsibilities\n

• Conduct thorough assessments and evaluations of patient status, utilizing the Diagnostic and Statistical Manual of Mental Disorders as needed to devise individualized care plans that incorporate cultural diversity and address specific patient needs.

• Utilize evidence-based practices to continuously assess patient progress and implement interventions that support mental health diagnoses, facilitating effective treatment outcomes.

• Document and maintain accurate treatment records and clinical data in compliance with regulatory standards and organizational protocols, utilizing advanced digital tools.

• Collaborate actively with patients and their families to develop comprehensive treatment plans that align with desired outcomes and ensure that patients’ voices are central to their care.

• Coordinate referrals and provide resource navigation to link patients with additional treatment options and community services, ensuring they receive holistic support.
