Although primarily remote, occasional engagement with staff may occur for team meetings or case consultations.

Key Responsibilities\n

• Utilize your education and expertise to independently assess and formulate psychosocial diagnoses, ensuring accurate treatment planning for each patient.

• Create individualized treatment plans that prioritize patient-centered approaches, aligning with the unique needs and preferences of individuals coping with serious illnesses.

• Implement evidence-based behavioral interventions such as solution-focused therapy, motivational interviewing, and problem-solving techniques tailored to suit individual patient circumstances.

• Actively coordinate care with multidisciplinary teams, including primary care providers, psychologists, and other healthcare professionals to ensure an integrated approach to patient care.

• Monitor and track patient treatment responses systematically, adjusting therapeutic approaches as needed to ensure progress towards established treatment goals.

• Support medication management protocols as outlined by clinical providers, focusing on adherence monitoring as well as evaluating side effects and overall efficacy of prescribed treatments.

The focus of this role is on patients in hospice care, many of whom are experiencing complex emotional and psychological challenges as they navigate end-of-life transitions. You will work closely with patients and their families, providing crucial support and resources that encourage dignity and comfort during this sensitive time. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• A current and valid license to practice as a Licensed Clinical Social Worker (LCSW) in the state of Kansas, free from restrictions.

Compensation and Benefits\n
