This role is structured as a remote position, providing the flexibility to conduct assessments and coordinate care plans from your home office while remaining connected with the hospice team. Although the position is primarily remote, occasional visits to the hospice center may be necessary to assess patient conditions and collaborate with healthcare providers. Standard work hours are Monday through Friday, from 8:00 AM to 4:30 PM, with possible evening or weekend hours based on client needs and emergencies.
