Key Responsibilities\n

• Perform ongoing assessments for clients to tailor interventions to their specific needs, ensuring comprehensive care.

• Communicate effectively with supervisors, direct care staff, and family members to discuss each client’s progress and treatment plans.
\n
• Draft a tentative schedule of appointments, including home visits and observations, to monitor client progress and engagement.
\n
• Facilitate home visits and in-home observations that contribute to creating a holistic view of the client’s situation.
\n
• Implement program plans effectively, ensuring that interventions are executed as designed to maximize client outcomes.
\n
• Actively participate in Interdisciplinary Team Meetings to discuss client care, share insights, and collaborate on treatment strategies.
\n
• Document progress through monthly and quarterly notes that capture client developments and evolve treatment strategies accordingly.
\n
• Organize and lead training sessions to ensure that staff are proficient in implementing program plans and maintaining accurate documentation.
\n
• Provide follow-up interviews and observations to foster ongoing client support and facilitate adjustment of care plans.
\n
• Maintain organized records to ensure that pertinent information is readily available for authorized personnel.
\n
• Serve as a liaison between our organization and the wider mental health community to foster collaborative efforts and development goals.

• Assist in developing training materials aimed at improving the quality of programs provided to clients.

This position focuses on serving a diverse clientele that includes both youth and adults. In this role, you will encounter a wide range of mental health conditions requiring sensitivity and individualized approaches. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Must possess an active and unrestricted license as a Professional Counselor in the state of Oregon.

• A minimum of 2 years of counseling experience in a clinical or similar setting is mandatory.

• Specialization in areas such as family therapy, couples therapy, substance misuse, or trauma is highly advantageous.

• A strong grasp of legal and ethical standards in counseling practice is essential.

• Demonstrated interpersonal and communication skills to establish rapport with clients and other professionals.

• Proven ability to work collaboratively with a multidisciplinary healthcare team.

• Competency in using Electronic Medical Record (EMR) systems and related technologies for documentation and client management.
