Key Responsibilities

• Conduct comprehensive evaluations of patients, which includes reviewing medical histories, performing physical assessments, and documenting evaluation findings to develop personalized treatment plans.
• Effectively communicate evaluation results and treatment plans to patients and their families, ensuring their understanding and involvement in goal setting.
• Implement treatment protocols tailored to individual patient needs based on assessment findings, using evidence-based practices in a fast-evolving healthcare landscape.
• Collaborate with other healthcare professionals to ensure comprehensive care, recognizing and documenting changes in patients' conditions that may require adjustments to their treatment plans.
• Manage discharge planning and follow-up care meticulously, ensuring documentation is timely and reflects accurate patient progress and treatment efficacy.
• Consistently meet productivity expectations while maintaining effective time management. Participate in hospital and community marketing efforts to promote the services offered at our center.
• Engage in the continuous evolution of departmental goals, participating actively in the alignment of our objectives with those of the broader organization.
• Pursue continuing education opportunities for professional development and serve as a mentor for students in clinical placements, fostering the growth of future practitioners.
• Commit to enhancing the patient experience through understanding and addressing obstacles to access to care.

Patient Population / Client Focus

The patient population at Swift Health Physical Therapy encompasses individuals recovering from a variety of conditions including orthopedic injuries, post-operative rehabilitation, neurological impairments, and chronic pain management. Our focus is to return patients to their highest functional level and improve their quality of life through personalized treatment approaches.

Qualifications

• A Bachelor's Degree in Physical Therapy is required; however, candidates with a Doctorate in Physical Therapy (DPT) are highly encouraged to apply.
• A valid Physical Therapist license in the State of New Jersey is mandatory.
• Current certification in Basic Life Support (BLS) from the American Heart Association is required.
• A commitment to continuous learning and adapting to new therapy innovations is essential for success in this role.

Preferred Experience

• While a minimum of 2 years of experience is preferred, new graduates with relevant clinical placements and strong educational backgrounds are also encouraged to apply.

Compensation and Benefits

The salary for this full-time position ranges from USD 100,000 to USD 120,000 annually, commensurate with experience.
