This position operates within a community health clinic environment, primarily functioning in a remote setting. Key Responsibilities\n

• Initiate and oversee discharge planning from the moment of patient admission, ensuring that individuals understand their coping strategies, transition plans, and available resources as they prepare for discharge.

• Conduct discharge sessions with patients and their loved ones to confirm follow-up appointments and ensure a smooth transition post-treatment.
\n
• Manage crises effectively in both individual therapy sessions and group settings, providing immediate support to patients and assisting milieu staff in their interactions.
\n
• Complete all clinical documentation in real-time during sessions to maintain compliance and uphold our standards of care.
\n
• Establish strong therapeutic rapport with clients and their families, conducting assessments for suicidality and risk of harm to self or others.
\n
• Facilitate seamless admission processes by conducting thorough assessments and maintaining a welcoming atmosphere for new patients.
\n
• Actively engage in weekly treatment team meetings, sharing patient progress and collaboratively developing clinical goals with interdisciplinary colleagues.
\n
• Perform utilization reviews for insurance providers, delivering accurate clinical information to secure necessary authorizations for patient treatment.
\n
• Maintain accessibility to family members by responding to communications within two hours during business hours and addressing other inquiries within 24 hours.
\n
• Ensure continuity of care by coordinating with referring providers, outpatient services, and conducting necessary communication during the admission, mid-treatment, and discharge phases.

Qualifications\n

• A Master’s or Doctorate degree in Clinical Psychology, Counseling, or Clinical Social Work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD) from an accredited institution.

• An active, independent license in the state of Oregon (e.g., LCSW, LMFT, LPC).
\n
• Proficiency in electronic communication and documentation practices, with experience utilizing electronic medical records systems.

• A demonstrated ability to manage a significant caseload while providing high-quality therapeutic services.

Compensation and Benefits\n
