Key Responsibilities\n

• Conduct thorough and ongoing assessments to address the unique needs of each client, utilizing evidence-based practices to inform treatment planning.

• Communicate regularly with supervisors, fellow counselors, family members, and other key stakeholders regarding clients' progress and necessary adjustments to their care plans.

• Provide detailed scheduling of appointments and home visits, ensuring that your time management aligns with the needs of your caseload.
\n
• Coordinate and conduct home visits to complete in-home assessments, chart reviews, and staff interviews, thereby enhancing service delivery.
\n
• Ensure all assigned program plans are implemented effectively and maintained according to organizational policies.
\n
• Participate in interdisciplinary team meetings, contributing insights that promote holistic treatment strategies and enhance collaborative care.

• Complete monthly and quarterly reports summarizing client progress to ensure transparency and accountability in treatment.
\n
• Lead training sessions for staff and team members on implementing program plans and maintaining accurate documentation.
\n
• Conduct follow-up interviews and maintain regular contact with clients to monitor ongoing needs and provide ongoing support.

• Serve as a liaison between the clinic and the broader mental health community, advocating for clients and promoting mental health awareness.

• Assist in developing and revising training materials aimed at improving service delivery and client outcomes.

Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field.
