Position Overview

Right Form Rehabilitation & Wellness Center in Carteret, New Jersey, is actively seeking a full-time, dedicated Physical Therapist (PT) to enhance our team of health professionals. Work Environment

You will work in an onsite outpatient office, which reflects our commitment to an integrated approach to health and wellness. The typical work schedule is Monday to Friday, from 8:00 am to 5:00 pm, promoting work-life balance while delivering quality patient care.

Key Responsibilities

• Assess and evaluate patients to create comprehensive treatment plans tailored to individual capabilities and challenges associated with physical limitations and disabilities.

• Implement appropriate treatment protocols based on patient evaluations and treatment progress, utilizing evidence-based methodologies that align with state regulations.

• Perform and document detailed patient assessments, including history reviews, specific evaluations, and the articulation of recommendations to guide therapy.

• Consult and collaborate with interdisciplinary healthcare teams to ensure cohesive patient care and make adjustments to treatment plans based on patient progress or changes in condition.

• Document patient care activities thoroughly, including initial evaluations, daily encounter notes, interim progress updates, and discharge evaluations, ensuring compliance with evolving healthcare standards.

• Engage actively in discharge planning while ensuring that follow-up care and rehabilitation needs are met efficiently.

• Participate in continued education opportunities, in-house trainings, and clinical services as mandated, remaining in compliance with local, state, and federal regulations.

• Prioritize patient safety and quality care, while maximizing departmental productivity through effective treatment delivery and thorough documentation practices.

• Attend regular team conferences and meetings to discuss patient care strategies and improve operational efficiencies within the therapy department.

Patient Population / Client Focus

Our patients comprise individuals recovering from surgeries, injuries, chronic pain, and various physical dysfunctions. You will work with diverse age groups, tailoring your therapeutic approaches to meet their specific rehabilitation needs. It is essential to not only evaluate physical capabilities but to foster a rapport that encourages patient engagement and motivation throughout their recovery journey.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.

• A valid state physical therapist license to practice in New Jersey.

• Strong interpersonal skills with a demonstrated ability to form therapeutic relationships with patients.

• Current CPR certification to ensure patient safety and emergency preparedness.

Preferred Experience

• Ideal candidates will have at least two years of experience in a relevant therapy setting, focusing on outpatient care.

• Experience with digital health solutions or personal training methodologies will be a valuable addition.

Compensation and Benefits
