Key Responsibilities\n

• Conduct in-depth psychosocial assessments through video consultations to identify the emotional, social, and environmental needs of patients and their families.

• Deliver evidence-based therapeutic counseling through virtual platforms, working with individuals, families, and groups to provide support and actionable coping strategies.

• Collaborate actively with physicians, nurses, and other healthcare providers to create and implement comprehensive, individualized treatment plans that respect the patients' goals and values.

• Advocate for patients’ rights, helping clients access essential community resources, palliative care services, and other support systems to enhance their quality of life.

• Document patient interactions, treatment plans, and progress in electronic health records, maintaining compliance with state and federal regulations.

Your work will primarily focus on clients facing end-of-life issues, including patients with terminal illnesses and their families. Qualifications\n

Compensation and Benefits\n

We offer a competitive compensation package that ranges from $115,000 to $130,000 per year , commensurate with experience and caseload.
