Key Responsibilities\n

• Provide individual counseling and emotional support, helping seniors navigate life transitions such as retirement, loss, or health changes.

• Advocate vigorously for clients' rights, ensuring they have access to necessary community resources and services that support their mental health.

• Perform regular follow-up visits to monitor clients’ mental and physical well-being, evaluate living conditions, and address safety or health concerns.

• Assist in resolving family conflicts regarding the care of senior clients, promoting stability and well-being within the family unit.

• Educate families about community resources, support groups, and programs designed to support their loved ones in maintaining social connections and enhancing their overall quality of life.

• Maintain meticulous documentation of client assessments, care plans, progress notes, and all relevant interactions to ensure continuity of care.

• Stay current with social work practices, ethical standards, and regulatory policies to ensure compliance in all aspects of service delivery.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Active and unrestricted state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of social work experience is essential, with preference given to those with experience in geriatric care or community-focused settings.

• Comprehensive knowledge of social work principles, community resources, and aging-related issues.

Compensation and Benefits\n

The salary range for this position is competitive, offering between $70,000 and $100,000 annually.
