Key Responsibilities

• Conduct thorough evaluations and examinations to determine patient treatment needs, developing personalized treatment plans for individuals with various functional limitations and disabilities.
• Utilize evidence-based practices to implement treatment plans effectively, continuously monitoring and evaluating patient progress and adjusting interventions as necessary.
• Provide clear consultations to healthcare providers regarding the continuum of care for our patients while ensuring adherence to established customer service standards.
• Maintain comprehensive documentation of treatment sessions and patient interactions, ensuring compliance with all applicable laws, regulations, and professional standards.
• Collaborate intimately with an interdisciplinary treatment team, engaging in meaningful discussions with referral sources and directly with patients and their families.
• Engage in community outreach initiatives and support continuous quality improvement (CQI) projects to promote health awareness and the benefits of physical therapy.
• Supervise and mentor assigned support staff, aiding in their professional development while effectively managing departmental resources to meet patient needs.
• Stay current in the field by participating in staff meetings, continuing education, and professional development opportunities.

Patient Population / Client Focus

The Physical Therapist will primarily work with a diverse patient population, including individuals recovering from orthopedic surgeries, sports injuries, chronic pain syndromes, and other conditions requiring rehabilitative support. Qualifications
