Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews, observations, and medical record reviews to evaluate patients’ emotional, social, and environmental factors affecting their recovery.

• Provide compassionate counseling and support to patients and their families, helping them navigate adjustment issues and enhancing emotional well-being during the rehabilitation process.

• Collaborate with rehabilitation team members to develop and adjust individualized treatment plans that prioritize the unique psychosocial needs of each patient.

• Assist patients in accessing crucial community resources, including support groups, vocational rehabilitation programs, and housing assistance services.

• Educate patients and their families about community resources and post-rehabilitation services to ensure continuity of care.

• Participate actively in discharge planning to facilitate smooth transitions for patients returning home or into community settings.

Patient Population / Client Focus\n

The primary focus of your work will be with individuals undergoing detoxification and rehabilitation, many of whom may be dealing with substance abuse issues and other co-occurring mental health disorders. You will play a vital role in addressing their emotional and practical needs, guiding them through their recovery process, and assisting them with reintegration into their community.

Qualifications\n

• Must hold a current and valid Licensed Clinical Social Worker (LCSW) designation.

• A Bachelor’s Degree in Social Work or a related field is required; a Master’s degree is preferred.
