Key Responsibilities\n

• Conduct thorough mental health assessments for patients, carefully evaluating their personal histories, presenting concerns, and potential risk factors.

• Design and implement individualized treatment plans in collaboration with patients and their families, focusing on mental health needs and facilitating a positive hospice experience.

• Utilize evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT) and trauma-informed care, to address diverse mental health challenges faced by patients.

• Facilitate both individual and group therapy sessions, utilizing engaging methods to promote emotional regulation, self-awareness, and coping strategies.

• Collaborate with a multidisciplinary team, including physicians, nurses, and counselors, to ensure comprehensive and coordinated care for patients and their families.

• Provide crisis intervention and support during mental health emergencies, employing de-escalation techniques and connecting patients with appropriate levels of care when needed.

• Educate patients and families about mental health conditions, treatment approaches, and available resources, empowering them to participate actively in their care.

• Maintain up-to-date and accurate documentation of assessments, treatment strategies, and progress notes, adhering to all legal and ethical standards.

Patient Population / Client Focus\n

This role focuses primarily on patients in a hospice setting, many of whom are dealing with terminal illnesses and complex emotional needs. You will provide emotional support to not only the patients but also their families, creating a safe space for them to navigate grief, loss, and the various stages of end-of-life care. This requires sensitivity, compassion, and an understanding of the diverse backgrounds and experiences of those we serve.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current and valid LCSW licensure in the state of Georgia.

Compensation and Benefits\n

The salary for this position ranges from $80,000 to $100,000 annually, commensurate with qualifications and experience.
