This position is primarily remote, allowing you to create a flexible work environment while contributing meaningfully to a community health clinic's mission. Despite the remote setting, you will engage collaboratively with a multidisciplinary team, actively participating in meetings and training via digital platforms. Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of children to determine their individual needs.

• Collaborate in the development and implementation of Individualized Family Service Plans (IFSP), ensuring that goal-setting aligns with family values and priorities.

• Track and monitor outcomes effectively as a member of a dedicated multidisciplinary team.

• Provide essential support and guidance to families, helping them understand their child's developmental journey.

• Maintain compliance with all Federal and State health and safety regulations to protect both staff and clients.

Patient Population / Client Focus\n

In your role as an LCSW, you will focus on an underserved population of infants and toddlers, providing support to children facing developmental delays or disabilities. Qualifications\n

Compensation and Benefits\n
