Clinical supervision available Responsibilities Assess the psychosocial functioning of Palliative and Home Health care patients and families in their home environment and develop an individualized plan of care Provide appropriate psychosocial interventions in accordance with the physician's orders and the policies and procedures of the Agency Educate patient/family on, and assist in, preparation of advance directives Provides information and referral to patients and families/caregivers regarding necessary community resources Maintains clinical records on all patients referred to social work prepares clinical and progress notes Function as an integral member of the interdisciplinary team Qualifications Master of Social Work Degree New Grads welcome, we will train the candidate; clinical supervision is available Bilingual in Spanish and English People-oriented self-starter; notable written and verbal communication skills, and customer service skills California licensed and insured driver with reliable transportation #J-18808-Ljbffr Haven-Hospice
