Work Environment

This position is primarily onsite, as you will conduct therapy sessions in the comfort of our clients' homes across Freehold, New Jersey. Your work environment includes various home settings, requiring adaptability and an ability to assess each patient's unique environment for rehabilitation assistance. You'll collaborate closely with an interdisciplinary team to deliver integrated care and consistently enhance the effectiveness of our services.

Key Responsibilities

• Direct physical therapy treatment to manage and prevent disability while relieving pain and restoring functional muscle capabilities.
• Oversee and guide Physical Therapy Assistants to ensure adherence to individualized care plans, conducting regular evaluations of treatment progress.
• Utilize diagnostic assessments to evaluate clients' physical abilities, incorporating tests on muscle, nerve, and joint functions.
• Conduct home assessments to recommend necessary modifications that will promote patient independence.
• Provide therapeutic exercises, manual therapies, and utilize physical agents such as traction and massage as indicated by physician prescriptions.
• Fit and adjust orthotic and prosthetic devices, recommending modifications to healthcare professionals as needed.
• Document all patient interactions, including initial evaluations, care plans, progress notes, and discharge summaries to maintain comprehensive clinical records.
• Participate in interdisciplinary team meetings, collaborating with other healthcare professionals to deliver coordinated treatment strategies.
• Educate patients and their families on physical therapy practices and the usage of assistive devices to empower them in their recovery process.
• Complete administrative duties including timely submission of treatment orders and documentation in accordance with company policies.

Patient Population / Client Focus

You will work with a diverse patient population, including individuals recovering from surgery, managing chronic illnesses, or rehabilitating from injuries. Our clients range in age, often requiring specialized care adjustments to meet their specific needs. Providing compassionate, individualized care is essential in fostering an environment conducive to healing and patient growth.

Qualifications

• Doctorate in Physical Therapy from an accredited institution.
• Current and valid physical therapy licensure in the state of New Jersey.
• Active CPR certification, ensuring a commitment to patient safety.
• A valid driver's license and automobile insurance for efficient travel to client homes.
• Strong interpersonal communication skills to engage effectively with patients and families.
• A collaborative mindset with a passion for leading patient care initiatives.

Preferred Experience

• A minimum of one year of experience in a physical therapy role, particularly within home care or acute settings.
• Demonstrated ability to make independent clinical decisions while maintaining high standards of care.
• Experience in developing and adjusting care plans based on patients' evolving needs.
• Proficiency in federal and state healthcare regulations to ensure compliance and best practices.

Compensation and Benefits
