Key Responsibilities\n

• Independently manage a diverse caseload of patients, conducting comprehensive assessments to understand and address the social, emotional, financial, and cultural factors affecting their healthcare journey.

• Lead and coordinate treatment plans that tackle complex psychosocial issues, ensuring effective crisis intervention services to support patients and their families during challenging times.

• Educate patients, families, and caretakers on available resources, facilitating referrals to community services and ensuring their needs are met for a successful treatment or discharge plan.

• Supervise and mentor a team of social work interns and volunteers, providing guidance to ensure optimal delivery of psychosocial services while fostering professional development among staff.

• Participate actively in care coordination activities, particularly for children with specific discharge needs, ensuring seamless collaboration with community agencies to address clinical issues and provide necessary support.

• Respond promptly to psychosocial needs, minimizing delays in implementing treatment plans or discharge timelines, and ensuring outstanding service and satisfaction for patients.

• Engage in continuous education and training for the healthcare team regarding the social work profession, enhancing awareness of psychosocial issues relevant to patient care.

Qualifications\n
