Key Responsibilities\n

• Conduct thorough ongoing assessments that address the unique needs of each patient, utilizing tools and methodologies designed for effective evaluation and progress tracking.

• Communicate regularly with supervisors, direct care staff, family members, and guardians to provide updates on client progress and treatment plans.

• Prepare detailed schedules of appointments, home visits, and provider observations to monitor patient advancement and treatment efficacy.

• Coordinate essential home visits and in-home observations, ensuring comprehensive care through chart reviews and collateral interviews.
\n
• Ensure that treatment plans are implemented effectively and modify them based on patient feedback and progress.
\n
• Participate in Interdisciplinary Team Meetings to discuss client care and treatment strategies, contributing to a collaborative approach to mental health.

• Document and complete required monthly and quarterly progress notes to maintain continuity of care.
\n
• Develop and organize training sessions to certify staff and team members on the proper implementation of treatment plans.
\n
• Provide ongoing support through follow-up interviews and continued assessments as necessary.

• Maintain accurate and organized records, ensuring that all information is readily accessible to authorized parties.

• Act as a liaison within the public and mental health communities to promote awareness and support development goals.

• Assist in creating educational materials and training procedures to elevate the quality of care for individuals served.

Qualifications\n
