Key Responsibilities\n

• Assist primary care providers in identifying and managing mental health and psychosocial issues, playing an essential role in the comprehensive care team.

• Conduct patient intakes, performing diagnostic and functional evaluations, and provide insightful recommendations for treatment goals and plans.

• Deliver brief, focused therapeutic interventions for patients seeking mental health support, customizing approaches based on individual needs.

• Advise primary care providers on appropriate referrals for patients who may benefit from specialty mental health facilities.
\n
• Initiate follow-ups to monitor patient progress and adapt treatment plans as necessary, ensuring continuous support for each individual's journey.

• Develop relapse prevention plans and assist patients in achieving and maintaining emotional stability.
\n
• Identify patients at risk and create proactive strategies to prevent deterioration in their mental health conditions.

• Coordinate and monitor the delivery of behavioral health services, ensuring seamless linkage with other treatment providers while respecting patient confidentiality.

• Document patient progress and diagnostic information accurately in treatment charts to maintain an organized and compliant record of care.

• Keep primary care providers informed of patient needs and progress, collaborating on comprehensive treatment plans.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s Degree in Clinical Counseling or a closely related field.

• Current active LPC (Licensed Professional Counselor) license recognized in Texas.

Compensation and Benefits\n

The starting salary for this full-time LPC position ranges from USD 87,000 to 105,000 annually, depending on experience and qualifications.
