Key Responsibilities\n

• Conduct thorough assessments on a continuous basis to effectively meet the varying needs of each patient.

• Maintain open lines of communication between supervisors, direct care staff, families, and other stakeholders to monitor individual progress.

• Develop and provide a structured schedule of appointments, home visits, and observations to ensure consistent and comprehensive care delivery.
\n
• Coordinate and conduct home visits for in-home assessments, facilitating chart reviews and staff interviews.
\n
• Oversee the implementation of individualized treatment plans, ensuring they are effectively operationalized to promote patient success.
\n
• Engage actively in Interdisciplinary Team Meetings and contribute insights related to patient care as required.
\n
• Document monthly and quarterly progress notes to track patient achievements and areas needing attention.
\n
• Design and organize training sessions for staff to ensure they are well-equipped to implement program plans and maintain accurate documentation.
\n
• Provide essential follow-up interviews and home visits to ensure ongoing support and guidance for patients.
\n
• Maintain organized and accessible records, ensuring information is readily available for all relevant parties.
\n
• Act as a vital liaison within the public and mental health communities, working toward shared goals of development and support.

• Assist in creating content and procedures that enhance training materials aimed at improving program effectiveness.

Qualifications\n

• Master’s or Doctoral degree in Counseling, Psychology, or a related discipline is required.

• Possession of an active and unrestricted Professional Counselor (LPC) licensure in the state of Colorado.

• A minimum of two years counseling experience in a clinical setting is essential.

• Specialization in family therapy, couples therapy, substance abuse, or trauma is highly desirable.

• Strong understanding of legal and ethical standards governing mental health practice.

• Exemplary interpersonal communication skills, with a proven ability to build rapport with a broad spectrum of clientele.

• Demonstrated capability to collaborate effectively as part of a multidisciplinary healthcare team.

• Proficiency in electronic medical record (EMR) systems and related technologies is highly beneficial.

Preferred Experience\n

Compensation and Benefits\n

The salary range for this position is competitive, ranging from USD 90,000 to USD 100,000 annually, commensurate with experience and caseload.
