Key Responsibilities\n

In this integral role, you will be responsible for:

• Conducting comprehensive patient intakes that prioritize diagnostic and functional evaluations to tailor treatment plans.

• Providing focused mental health interventions for individuals experiencing mental health crises or challenges.
\n
• Collaborating with primary care providers to assist in recognizing, treating, and managing psychosocial and mental health issues.

• Developing and implementing personalized relapse prevention plans to help clients maintain stability.
\n
• Documenting patient progress and maintaining detailed treatment charts in accordance with agency guidelines.
\n
• Monitoring health services delivery related to behavioral health, linking with other treatment providers both within and outside the clinic.

• Ensuring patient compliance with recommended medical treatments and addressing any side effects of medications.

• Conducting follow-ups to evaluate patient outcomes and adjust treatment plans as necessary.

• Counseling patients on behavioral changes to improve overall health and lifestyle choices.

Patient Population / Client Focus\n

You will engage with a diverse patient population, including individuals facing various mental health challenges connected to their physical health or socio-economic backgrounds. Qualifications\n

To succeed in this role, candidates must possess the following qualifications:

Compensation and Benefits\n

Mountain Springs Wellness & Counseling Services offers a competitive salary ranging from $87,000 to $105,000 annually, commensurate with experience.
