Key Responsibilities

• Conduct comprehensive evaluations of patients, gathering relevant medical histories and performing specific assessments to develop effective treatment plans.
• Communicate evaluation findings and treatment strategies clearly with patients and their families, actively involving them in goal-setting processes.
• Implement tailored treatment protocols based on initial evaluations, patient progress, and emerging healthcare practices.
• Collaborate with interdisciplinary teams of healthcare workers to address complex patient needs and adapt care plans as necessary based on observed changes in patient conditions.
• Document patient care activities meticulously, ensuring timely and accurate record-keeping of evaluations, treatment progress, and discharge planning to meet healthcare compliance standards.
• Manage your schedule effectively to meet productivity targets while fostering patient accessibility and a pleasant therapeutic environment.
• Engage in community outreach and marketing efforts to promote physical therapy services and educate the public on wellness.

Patient Population / Client Focus

Your primary focus will be on patients receiving physical therapy in their homes, ranging from post-operative recovery to chronic pain management. You will develop a rapport with patients and their families, tailoring your therapeutic approach to fit each individual's lifestyle and support network, enhancing both recovery outcomes and overall quality of life.

Qualifications

• A Bachelor's degree in Physical Therapy from a recognized institution.
• Graduation from an accredited Physical Therapy program is required.
• A valid state license to practice physical therapy in Virginia or the eligibility to obtain one.
• Current Basic Life Support (BLS) certification for Healthcare Providers from the American Heart Association.
• No prior work experience is required; however, demonstrated school-based clinical internship experience is beneficial.

Preferred Experience

• Experience or internships involving home health care or outpatient physical therapy is a plus.
• Familiarity with electronic medical records (EMR) systems and documentation practices.
• Evidence of participation in continued education workshops or relevant courses.
