Key Responsibilities\n

• Conduct comprehensive assessments to ascertain the unique needs of each patient, tailoring therapeutic approaches accordingly.

• Maintain open lines of communication with supervisors, other direct care staff, and clients' families, providing updates on progress and integrating feedback to refine care strategies.

• Develop and provide a tentative schedule detailing appointments, home visits, and progress monitoring sessions to ensure thorough follow-up on each client's treatment plan.

• Coordinate home visits and in-home observations to gather real-time information on patient progress and adjust care strategies as necessary.

• Ensure all aspects of the program plan are effectively executed and regularly assessed for efficiency and efficacy.

• Actively participate in Interdisciplinary Team Meetings, contributing insights and collaborating on individual care plans as needed.

• Document therapy sessions and client progress through meticulous monthly and quarterly notes, ensuring accuracy and compliance with all relevant regulations.

• Organize and lead training for staff to enhance their competency in implementing program plans and maintaining proper documentation protocols.

• Provide ongoing support through follow-up interviews and additional home visits to ensure that individual client needs are being met and adjusted over time.

• Act as a liaison with the broader mental health community, facilitating collaboration and providing support for community development goals related to mental wellbeing.

• Contribute to creating and refining training materials aimed at improving the quality of our programs for individuals of all demographics.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.
