Key Responsibilities\n

• Conduct comprehensive assessments to identify the counseling needs of clients, focusing on mental health and substance use disorders.
\n
• Utilize DSM (Diagnostic and Statistical Manual) criteria to accurately diagnose clients and determine their appropriate level of care using ASAM (American Society of Addiction Medicine) criteria.
\n
• Prepare and develop substance abuse counseling components for individualized treatment plans, tailored to each client’s unique circumstances.
\n
• Create effective discharge and continuum of care plans to facilitate smooth transitions between different levels of service, ensuring ongoing support.
\n
• Document all counseling sessions, education services, and assessments meticulously within clients' clinical records to maintain quality care and compliance.
\n
• Manage a caseload that includes treatment planning and clinical charting, in addition to case management and utilization reviews as required.
\n
• Coordinate support services for clients and their families, recording these interactions in the client's clinical record
\n
• Engage and collaborate with referral sources such as legal representatives, educational institutions, and employers to provide comprehensive support.
\n
• Facilitate crisis interventions for clients experiencing urgent issues, utilizing a multi-disciplinary team approach to resolve complex situations effectively.

The ideal candidate will work with a diverse client base, including individuals facing substance use disorders, mental health challenges, and those navigating the delicate and sensitive aspects of hospice care. The LCSW plays a vital role in ensuring that patients receive holistic treatment, supportive counseling, and compassion-driven care at every stage of their recovery journey.

Qualifications\n

• A Bachelor's degree in Social Work is required; a Master's is preferred, along with LCSW licensure in Texas.

• Demonstrated self-motivation and self-sufficiency in managing responsibilities efficiently in a remote work setting.

Preferred Experience\n

• Experience providing counseling for co-occurring disorders is highly desirable.

• Experience working within hospice settings or knowledge of end-of-life care practices is an asset.

Compensation and Benefits\n

We offer a competitive compensation package, with an annual salary ranging between USD 70,000 and 100,000, depending on experience and qualifications. Benefits include:
