Key Responsibilities\n

• Conduct comprehensive bio-psychosocial assessments for patients in the hospice care setting, ensuring cultural competence and adherence to best practices.

• Facilitate individual and group counseling sessions, addressing a variety of concerns such as grief, anxiety, depression, and coping with terminal illness.

• Document mental health evaluations, treatment plans, and progress notes electronically in compliance with legal and ethical standards.

• Conduct suicide risk assessments and escalate patient needs appropriately by collaborating with medical providers to develop client-focused solutions.

• Act as a liaison between patients and community resources, including social services and medication assistance programs, ensuring patients and their families receive necessary support.

• Engage in quality improvement initiatives by collaborating with team members to enhance service delivery and patient outcomes.

Patient Population / Client Focus\n

The LCSW will work primarily with terminally ill patients and their families, providing emotional support and counseling to address complex psychosocial issues. Qualifications\n

• Active Licensed Clinical Social Worker (LCSW) license in California is mandatory.

Compensation and Benefits\n
