Key Responsibilities\n

• Conduct thorough assessments of clients' social, emotional, and environmental challenges to inform tailored service plans.

• Develop comprehensive service plans that outline individualized goals, necessary interventions, and a framework for monitoring progress.

• Provide insightful counseling and support to individuals and families navigating life’s complexities, ensuring that interventions are culturally competent and appropriate.
\n
• Engage in crisis intervention efforts, offering risk assessments and developing safety plans when necessary, particularly for at-risk clients.
\n
• Collaborate with clients to connect them with relevant community resources and services that augment their support networks.
\n
• Participate actively in case conferences and team meetings to enhance service delivery and consistency across care interventions.

• Engage in community outreach initiatives to raise awareness of our agency's services and advocate for systemic change.

Patient Population / Client Focus\n

In this role, you will work primarily with individuals and families facing a spectrum of social challenges, including but not limited to homelessness, domestic violence, substance abuse, and poverty. Your efforts will contribute to empowering these populations through advocacy and resource facilitation, ultimately aiding them in achieving stability and improving their quality of life.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• Possession of an active state licensure as a Licensed Clinical Social Worker (LCSW).

• Strong understanding of social and economic justice issues, trauma-informed care, and community resources.
\n
• Excellent communication skills, cultural competence, and the ability to approach problem-solving with creativity and sensitivity.

Preferred Experience\n

• Previous experience working in social service agencies or similar settings is preferred, with a demonstrated focus on direct client care.

Compensation and Benefits\n
