Work Environment\n

This position is full-time with a standard work schedule from Monday to Friday, 9 AM to 5 PM. Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs and develop individualized care plans that align with their goals and preferences.

• Actively participate in multidisciplinary team meetings to discuss patient progress, share insights, and collaboratively adjust care strategies.

• Provide education and support to clients and their families, empowering them with tools and coping strategies to navigate healthcare challenges.

• Utilize evidence-based practices and therapeutic methodologies to address grief, loss, anxiety, and depression.

• Develop and maintain strong working relationships with community resources, therapeutic agencies, and support groups to enhance client care.

Patient Population / Client Focus\n

Your primary focus will be on patients in hospice care, which requires an empathetic understanding of the complexities surrounding end-of-life decisions. You will support clients from diverse backgrounds, ensuring culturally sensitive practices that respect their values and beliefs. Your role is critical in helping families process grief and make informed choices regarding their loved ones' care, thus improving overall patient outcomes.

Qualifications\n

• A Bachelor’s degree in Social Work from an accredited institution is required; a Master’s degree is preferred.

• Current New Jersey LCSW licensure is a must.

Compensation and Benefits\n
