Work Environment\n

This position is performed in a remote setting, allowing flexibility and convenience while fostering an innovative approach to outpatient mental health care. As part of our outpatient office, you will engage in meaningful interactions with patients and professionals while utilizing electronic medical record (EMR) software and other digital tools for streamlined documentation and communication.

Key Responsibilities\n

• Conduct comprehensive and ongoing assessments to identify the needs of each patient, ensuring tailored treatment plans.

• Maintain clear communication with supervisors, direct care staff, family members, and other involved parties regarding an individual's progress and any adjustments in treatment strategies.

• Draft and provide a tentative schedule for appointments, home visits, and observations in order to monitor progress effectively.
\n
• Coordinate home visits that may involve in-home observations, chart reviews, and staff interviews to gather necessary data for analysis.
\n
• Oversee the implementation of program plans, ensuring they are executed efficiently and effectively.
\n
• Participate in interdisciplinary team meetings, contributing valuable insights and collaborative strategies for patient care.
\n
• Complete detailed monthly and quarterly progress notes to document patient progress and treatment outcomes.
\n
• Facilitate training sessions to ensure staff and team members are fully equipped and certified to implement the program plan and documentation standards.
\n
• Conduct follow-up processes through interviews and home visits to provide continuous support tailored to individual needs.
\n
• Maintain accurate and thorough records, making necessary information readily accessible to relevant stakeholders.
\n
• Act as a liaison between the public and the mental health community to support shared development goals and effective care.

• Assist in developing training materials and procedures that aim to enhance program effectiveness for the individuals served.

Patient Population / Client Focus\n

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.

Compensation and Benefits\n
