Key Responsibilities\n

• Conduct comprehensive assessments regularly to identify and address the evolving needs of clients, ensuring personalized treatment plans are maintained.

• Act as a primary communication link among supervisors, direct care staff, family members, and other stakeholders to provide updates on client progress and facilitate coordinated care.

• Create and provide supervisors with a proposed schedule for appointments, home visits, and client observations for monitoring treatment progress effectively.
\n
• Plan and coordinate home visits, as required, for observations, data collection, and interviews that align with our comprehensive approach to client care.
\n
• Ensure effective implementation of program plans, regularly reviewing methodologies to confirm they are effectively meeting the needs of clients.
\n
• Participate actively in Interdisciplinary Team Meetings to discuss ongoing cases and contribute to treatment planning.
\n
• Maintain accurate and timely records, including detailed monthly and quarterly progress notes that provide insights into each client's journey.
\n
• Organize and conduct training sessions for staff on the implementation of program plans, ensuring all team members are equipped with the necessary skills and knowledge.
\n
• Provide follow-up interviews, home visits, and continuous observations to enhance client support based on individual requirements.
\n
• Act as a liaison between the organization and the community to optimize support networks and facilitate client progression towards developmental goals.

• Assist in creating and updating training materials for staff, aimed at improving program delivery and efficacy.

As a Licensed Professional Counselor with us, you will work with a varied client population, including adults, youth, families, and individuals facing challenges related to trauma, substance abuse, and relationship issues. Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• An active and unrestricted license as a Professional Counselor in the state of Texas.
