Key Responsibilities

• Conduct thorough evaluations of residents upon admission, assessing their physical capabilities and establishing treatment goals that align with their rehabilitation needs.
• Design and implement individualized treatment plans that address mobility, strength, and functional independence, utilizing various therapeutic modalities such as manual therapy and therapeutic exercises.
• Administer targeted therapeutic interventions and educate residents on exercises that can be practiced beyond the treatment sessions, helping them gain control over their recovery.
• Regularly monitor and document patient progress, making necessary adjustments to treatment plans based on ongoing assessments and objective findings.
• Collaborate with a multidisciplinary team to enhance patient care by sharing insights and participating in care conferences to ensure a unified approach to treatment.
• Provide education to patients and their families about the recovery process and strategies to maintain their physical health, empowering them with knowledge and resources.

Qualifications

• A valid license to practice as a Physical Therapist in New York State is required.
