Work Environment

This position is based onsite as part of our home care service, where you will work closely with patients in the comfort of their own living environments. Our dedicated care team consists of experienced Physical Therapists and a Physical Therapy Assistant, creating a collaborative atmosphere that fosters professional growth and patient-focused results. The work schedule is Monday through Friday from 9 AM to 5 PM, allowing for a balanced work-life integration.

Key Responsibilities

• Effectively assess and understand the goals and needs of each patient, creating tailored treatment plans based on individual assessments.
• Manage a patient caseload that allows for in-depth, personalized care, typically servicing one patient per hour.
• Utilize your background in orthopedic outpatient care to provide evidence-based treatments designed to improve patients' mobility and overall function.
• Minimize reliance on medications, advocating for and providing non-surgical treatment alternatives.
• Develop and implement a variety of rehabilitation strategies, incorporating innovative and effective care techniques.
• Customize fitness and wellness plans that align with each patient's specific requirements, focusing on enhancing their strength, flexibility, balance, and coordination.
• Engage and motivate patients throughout their recovery journey, continuously monitoring their progress and adjusting treatment plans as necessary.
• Collaborate with other healthcare professionals, ensuring holistic care and communication regarding patient status and treatment goals.
• Document all patient interactions, treatments, and care plans accurately to maintain comprehensive patient records.

Patient Population / Client Focus

Our primary client base consists of individuals with private medical insurance, allowing us to offer comprehensive and accessible services. We take pride in serving a diverse group of patients, including those recovering from orthopedic surgeries, injuries, or related conditions. Your role is vital in guiding these patients toward improved physical health and independence.

Qualifications

• Possession of a Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current licensure as a Physical Therapist in New York state with no disciplinary actions on record.
• Strong background in orthopedic physical therapy, with a focus on manual therapy techniques and patient-specific exercise regimens.
• Proficiency in electronic medical record (EMR) software, preferably WebPT, for efficient documentation and record-keeping.
• Ability to communicate effectively with patients, team members, and other healthcare providers.

Preferred Experience

We are looking for candidates with at least two years of experience in orthopedic outpatient care. Experience in a home care setting or familiarity with patient-centered care models is preferred, as these skills will enhance your ability to connect with patients and tailor your approach to their unique needs.

Compensation and Benefits

The annual salary for this position ranges from $100,000 to $120,000, commensurate with experience.
