Key Responsibilities\n

• Plan and implement individualized support packages through comprehensive assessments to address clients' specific challenges and assist them in overcoming dependencies.

• Conduct in-depth interviews with service users, evaluating their current condition as well as identifying their strengths, needs, and areas for growth.

• Formulate tailored measurable goals for each case, using a person-centered approach to foster personal development within agreed-upon timelines.

• Regularly monitor and document clients' progress, adjusting treatment plans as necessary to align with evolving needs and goals.

• Provide professional counseling and evidence-based recommendations during sessions to guide clients toward the most appropriate actions.

• Maintain detailed and confidential records of client interactions, emphasizing ethical standards and accuracy in reporting.

• Participate in case conferences, and if necessary, provide expert testimonies when cases are referred to court.

• Serve as a key-worker, collaborating effectively with multidisciplinary teams to streamline processes and improve client outcomes.
