Key Responsibilities Communicate with physicians and other healthcare professionals to report on patient progress, addressing any needs and concerns that may arise during treatment. Maintain accurate clinical records, including timely clinical and progress notes as per departmental policies, ensuring compliance with healthcare regulations. Collaborate with the healthcare team to develop individualized treatment plans that reflect the psychosocial assessments, leveraging recognized social work interventions. Provide counseling and crisis intervention to patients, conducting assessments and addressing emotional and behavioral challenges effectively. Assist clients with discharge planning, facilitating connections with various service providers in the region to ensure access to necessary resources post-treatment. Coordinate closely with families and support systems to ensure comprehensive understanding and planning for client treatment and discharge. Patient Population / Client Focus As an LPC at Pathways Wellness Center, you will work with a diverse patient population, which includes individuals facing various mental health challenges such as anxiety, depression, trauma, and substance abuse issues. Your role is crucial in helping these individuals navigate their mental health journeys, focusing on holistic methods that incorporate their unique backgrounds and experiences. Qualifications A Master’s degree in Professional Counseling from an accredited institution is required. Current licensure as a Licensed Professional Counselor (LPC) or equivalent is essential for this role. A minimum of two years of full-time experience providing direct mental health services is required for candidates. Demonstrated ability to work independently, showcasing self-direction and exceptional organizational skills. Proficiency in computer skills, particularly with Microsoft Office suite, is necessary for documentation and communication purposes. Preferred Experience Experience with telehealth platforms is advantageous, as our team incorporates technology into client care delivery. A background in community health settings or experience working within a collaborative healthcare environment is preferred. Knowledge of assessment tools and therapeutic modalities that support diverse client needs will be beneficial.
