Key Responsibilities\n

• Conduct comprehensive assessments of patients' psychological and emotional needs through telehealth sessions, employing state-of-the-art digital assessment tools.

• Provide supportive counseling services tailored to individuals seeking transplant or other critical health services, ensuring they feel heard, understood, and guided throughout their journey.

• Educate patients on available community resources, such as vocational rehabilitation and home health services, to facilitate optimal treatment outcomes and improve their quality of life.

• Engage in continuous professional development to refresh your knowledge of current local vocational/educational rehabilitation programs and share this information with patients as needed.

• Assist patients in understanding and discussing advance directives, collaborating with healthcare teams and families to respect and implement patient wishes.

• Participate actively as a member of the interdisciplinary care team, bringing insights into patients' psychosocial status and contributing to comprehensive care planning.

• Utilize established social work theories and methodologies to inform care planning, identify barriers, and improve treatment goals through structured patient education programs.

• Assist patients with adjusting to new life circumstances, such as adapting to dialysis, by providing ongoing monitoring and targeted interventions.

Qualifications\n
