Key Responsibilities\n

• Conduct thorough and ongoing assessments tailored to the individual needs of your clients, ensuring their mental health care is personalized and effective.

• Serve as a crucial point of communication between supervisors, direct care staff, and the clients’ family members to facilitate a comprehensive understanding of each client’s progress.

• Create and share a tentative schedule for client appointments, including follow-up home visits and direct observations to enhance therapy outcomes.

• Coordinate meticulous home visits that encompass in-home observations, chart reviews, and staff interviews, while applying analytical skills to interpret client data.

• Oversee the implementation of therapeutic program plans, ensuring they are executed efficiently and effectively to maximize client satisfaction and success.

• Participate in Interdisciplinary Team Meetings as well as other pertinent meetings, contributing insights and updates on client progress and therapy outcomes.
\n
• Document and complete monthly and quarterly progress notes to maintain a clear and accurate record of client journeys.
\n
• Organize training initiatives for staff members to ensure competence in the application of program plans and maintain high standards of care.
\n
• Facilitate follow-up interviews, home assessments, and observational sessions in order to ensure continued support and adaptability to client needs.

• Act as a key liaison with the public and mental health community, fostering relationships to assist with development goals and outreach.
\n
• Contribute to the development of training materials and procedures aimed at enhancing program effectiveness and staff competency.

Patient Population / Client Focus\n

Qualifications\n
